Read the following waiver and respond 'YES' below:

Waiver and Release of Liability

In consideration of being permitted to participate in personal training services provided by FollowNaturalFitness (hereinafter referred to as "FollowNaturalFitness"), the undersigned (hereinafter referred to as "the Participant") agrees to the following:

1. 1. Assumption of Risk
      The Participant acknowledges that engaging in physical exercise and training involves inherent risks, including but not limited to, the risk of injury, illness, or other adverse effects. The Participant voluntarily assumes all risks associated with participation in the personal training services provided by the Company.
   2. Release of Liability
      To the fullest extent permitted by Connecticut law, the Participant agrees to release, discharge, and hold harmless the Company, its owners, employees, agents, and representatives from any and all claims, demands, actions, or causes of action for any injury, illness, or damage arising out of or related to the Participant's involvement in the personal training services. This release of liability extends to any claims based on negligence, whether active or passive.
   3. Medical Clearance
      The Participant affirms that they are in good health and physically capable of participating in the personal training services. The Participant agrees to consult a physician prior to beginning any exercise program if they have any pre-existing health conditions or concerns. The Participant also agrees to notify the Company of any medical conditions or limitations that may affect their participation.
   4. Indemnification
      The Participant agrees to indemnify and hold harmless the Company, its owners, employees, agents, and representatives from any and all claims, liabilities, damages, costs, and expenses, including but not limited to attorney’s fees, arising from or related to any injury or damage sustained by the Participant during or as a result of participation in the personal training services.
   5. Acknowledgment of Understanding
      The Participant acknowledges that they have read this Waiver and Release of Liability, understand its contents, and voluntarily agree to its terms. The Participant acknowledges that this waiver is a legally binding contract and has had the opportunity to consult with legal counsel before signing.
   6. Governing Law
      This waiver shall be governed by and construed in accordance with the laws of the State of Connecticut, without regard to its conflict of law principles.

By signing up, you agree to the following:

I have hereby enrolled in a program of strenuous physical activity including but not limited to aerobic dance, weight training, and various conditioning from FollowNaturalFitness. I hereby affirm that I am in good physical condition and do not suffer from any disability that would limit or prevent by participation in this exercise program.
In consideration of my participation of FollowNaturalFitness exercise program, I, for myself, my heirs and assigns, hereby release FNF from any claims, demands and causes of action arising from my participation in the exercise program. I fully understand that I may injure myself as a result of my participation in FNF's exercise program and I hereby release FollowNaturalFitness from any liability, now or in the future, however caused, occurring during and or after my participation in the exercise program.
